Improving the Clinical Trial Experience: Why, How, and What?

Global clinical trials are a vital component of pharma R&D. Utilizing sites in multiple countries not only provides increased access to eligible and diverse patients - and therefore more robust data - but also promotes health equity and research and innovation across continents.

Another benefit of global clinical trials is the potential for these studies to be completed more quickly and efficiently [1]. However, around four in five clinical trials fail to meet their patient recruitment deadlines [2], which delays patient access to innovative new therapeutics and can prove costly to Sponsors.

Just as treatments are becoming more personalized, so should clinical trial patient recruitment strategies. A ‘one size fits all’ approach to enrollment fails to consider how factors such as an individual country’s culture, healthcare structure and patients’ needs can affect a site’s ability to recruit eligible participants. Regulator Roundtable: regulating in a rapidly evolving world

Over the last few years, a long-running debate has been taking place about what are the appropriate approaches to assessment for today’s world and, since the conventional assessment of GCSEs and A levels was suspended during COVID-19, this debate has intensified with calls to replace GCSEs.  

Additionally, the pandemic accelerated the development of the digital classroom, and so, digital, or e-Assessments, are now on the horizon. Just what this will look like is open to question. For instance, AQA’s trial of digital GCSEs is exploring adaptive – or ‘smart’ – assessments that adjust in difficulty as students progress through the test. The International Baccalaureate (IB) has been assessing learning digitally since 2016 in its curriculum for 11 – 16 year olds, the Middle Years Programme (MYP), and has taken an alternative approach which utilises the full potential of the digital environment to assess knowledge and skills and to enhance learning in the classroom through a positive ‘backwash’ effect.

Moving towards eAssessment with the Middle Years Programme (MYP)

We have seen a growing focus on patient diversity in clinical trials in recent years, but the COVID-19 vaccine trials in particular have highlighted the importance of achieving diverse patient recruitment.

Between 2008 and 2018, fewer than two in three (63%) of the 230 clinical trials that led to cancer drug approvals by the Food and Drug Administration (FDA) in the US, recorded participants’ ethnicity. This lack of diversity is a major issue: studies show that different groups can respond to treatments in various ways, resulting in some medicines being prescribed differently.

This session will highlight some of the factors underpinning the lack of patient diversity in clinical trials, investigate the implications this has for patients, and discuss what we can do in practice to improve diversity in clinical trials.
